Community Heath Assessment and Planning: Assessment and planning can help strategize internally and externally to make the most of our efforts to keep our community healthy and thriving. Assessment and planning includes community health assessment and planning (CHA, CHIP) to help determine local public health and community priorities, and organizational and strategic assessment and planning to determine priorities and actions.
